The FCS program helps people with complex health needs get and keep stable housing through Apple Health (Medicaid). This program is for those who might not succeed in housing without extra support.
Why It Matters
Homelessness can lead to serious physical and mental health problems, as well as substance abuse. The FCS program aims to break this cycle by offering home- and community-based services across the state.
Supportive Housing Services are designed to help people who are homeless or at risk of homelessness live independently. These services include:
Before You Move In (Pre-tenancy Supports):
Assessing your housing needs and preferences
Helping you budget for housing and living expenses
Connecting you with social services for filling out applications and obtaining income sources
Assisting you in finding and applying for housing
Creating a personalized community integration plan
Setting and working towards short and long-term goals
Participating in planning meetings to review and adjust your needs
After You Move In (Tenancy-sustaining Services):
Supporting you in planning and participating in regular review meetings
Linking you to various services like healthcare, substance use treatment, mental health support, education, and employment
Helping you with paperwork and coordinating with entitlement agencies
Assisting with counseling, support groups, and skill-building for independent living
Helping you communicate with landlords about your needs and emergency procedures
Reviewing and updating your housing support plan regularly
Providing training and resources to help you be a good tenant and manage your household
